About Us

Home Health Care Partners is co-sponsored by St. Mary's Hospital and Nathan Lattauer Hospital, and is a not-for-profit agency licensed by New York State to provide home care services. We serve those persons whose care needs are not eligible for coverage under Medicare and Medicaid.

While providing both professional and paraprofessional services, Home Healthcare Partners collaborate with physicians, health care facilities, senior living complexes and other home care agencies to enhance and provide home care services when needed. Our registered professional nurses, licensed practical nurses, home health aides, companions and sitters, as well as all of our services, are available 24 hours a day.

Our staff is carefully screened prior to employment. Training as required by New York State Department of Health is provided by our nursing staff on a regular basis. Monthly educational programs are provided to help to assist home health aides to remain current with home care strategies and needs. Annual competency evaluations are held for each staff person in a clinical lab setting. Supervision of staff takes place in the home in a care setting.

Registered professional nurses establish the care plan which provides direction for the staff providing care in the home. The nurse is available 24 hours a day to answer questions and provide direction to the patient/client, family and staff.
